Let's Get Started...

We know living independently at home is very important. This is your safe zone, your comfort zone, the place where memories were created, shared, but most importantly is where you want to be.  We’ll work with you to find a caregiver who can help ease the burden of everyday tasks, ensure a fuller, and more dignified quality of life.

Initial Phone Call

During our initial call, you will speak to one of our staff members who will be your main point of contact throughout the intake process. Once you have provided the information regarding the type of service needed or just needing a good listening ear our staff member will collect general information from you.  This staff member will tell you about Melody Home care, who we are, what we do and how we can be of assistance.

An appointment will be made for a FREE, no-obligation home assessment. 

​

Our  Care Service Coordinators will come to your home and meet with you and your family to determine what type of service is needed.

During The Initial Agency Visit

During our visit we will take the time to learn as much as possible about your needs, home care issues, and the qualities of a Caregiver you seek.​​ From this point  if you and/or your family decide to proceed we will schedule for our nurse to come out to conduct the intake assessment which includes: 

The Assessment requires no obligation on your part.  You will not be asked to sign anything at that time unless you wish to proceed with services.

The Intake Assessments Include:

  • General background information

  • Medical and Medication Profile

  • Home Safety Inspection

  • Level of Function

Once the intake process has been completed; the nurse will take the information gathered during that assessment to develop a "Plan of Care"

Now It's Time To Match

Based upon the information provided during our initial call and the intake assessment we will begin to determine who will be a match or best fit for your needs.  We match our families together based upon personality, level of experience, certifications just to name a few so that it is a good marriage for everyone.

Meet And Greet

Once a caregiver has been identified, our Cares Service Coordinators will explain everything about your case to the new Caregiver. In addition, our Care Service Coordinators will come to your home to introduce you and to familiarize and orient your Caregiver to the Plan of Care, your home environment, and the social dynamic. This increases the likelihood of a comfortable transition. The introduction is very important in toward establishing a successful relationship between you and your Caregiver. The first few days/weeks are crucial to the relationship’s long-term success. This is not a common practice, and we believe is indicative of the passion we have for our work.

Ongoing Supervision

Our Nurse and Care Service Coordinators will conduct quarterly supervisory home visit to ensure the Plan of Care is being properly followed or needs any revisions; along with following up on how well you and the caregiver are getting along.  This is another major differentiator for our home care service.
